Assistant Team Manager - South and Central Hospital Discharge Team

Hours: 27.75 hours a week
Contract: 1 year fixed term contract/secondment
In addition to the advertised salary, you will receive a market premium of £525 per annum.

About the role
Adult Social Care Assistant Team Managers will support Team Managers in the day-to-day operational management of the following key areas:
- Developing a strength-based culture in practice and leadership/coaching of the team
- Ensure team compliance with all statutory social care requirements including the Care Act, the Mental Health Act, the Mental Capacity Act and Buckinghamshire Multi-agency Safeguarding procedures
- Contribute to service improvement to deliver outstanding support to Buckinghamshire residents
- Performance against statutory and locally determined indicators
- Provide leadership, supervision, and development opportunities to the team.
- Manage team performance, handle complaints, and maintain business continuity plans.
You will provide leadership and motivation to the team and ensure that staff work within directorate policies, procedures and guidelines to ensure the highest standards of service are provided to the residents of Buckinghamshire.
Mobile:
A ‘mobile’ role gives you the flexibility to schedule your own diary, so you can meet with colleagues and clients in a variety of locations. You’ll access digital folders and resources on your laptop and be able to use the network wherever you are based. Working from home will be common, as well as meeting your team in the office where necessary.
There is an expectation that the successful post holder will work from the office 2 days a week.
About you
Naturally, you will be a great people person, familiar with working to a strength-based approach where you think outside of the box and enable people to prevent, reduce or delay the need for care. Bringing your in-depth knowledge of the latest legislation and statutory guidance, you’ll work independently and with colleagues to support adults and their families. Persistence and an adaptable approach is needed in an outcome focused way with partners such as health services, community sector, housing, care providers and other stakeholders.
