Operations Coordinator – Home Independence Team

Contract: Secondment/fixed term contract until 9th February 2026

About us

The Home Independence service supports individuals to regain their independence following illness, disability, or recent stay in hospital. We work with clients in their homes, identifying whether they now have difficulties with self-care or performing daily activities. We provide a goal-based assessment for each client and alongside them to support the achievement of their goals, using technology-enabled care devices and occupational therapy aids to enable them to achieve independence.

The team works closely with healthcare professionals, carers, clients and their families to develop suitable packages of support, designed to enhance client independence and meet their needs. We supply advice on specialist equipment options, monitor client progress and make necessary adjustments if required. About the role

As Operations Coordinator, you will:

  • Lead and manage staff across the Home Independence Team.
  • Oversee and supervise Deputy Coordinators in the Home Independence Team.
  • Coordinate referrals, reviews, and care planning, ensuring services meet client needs and quality standards.
  • Manage operational resources, recruitment, induction, and HR processes.
  • Ensure compliance with CQC and internal policies and monitor performance against agreed indicators.
  • Promote independence and wellbeing, including supporting clients into employment where appropriate.
  • Liaise with clients, families, and partner agencies to ensure effective support and resolution of complaints.
  • Deliver and evaluate training, support team development, and contribute to service improvement.
  • Participate in an on-call rota and support seven-day working where required.

You’ll be instrumental in ensuring services are proactive, personalised, and delivered in line with our Better Lives framework.

About you

You’ll bring:

  • Substantial experience in adult social care or health services.
  • Strong leadership and coordination skills, with the ability to manage staff and resources effectively.
  • In-depth knowledge of relevant legislation, including the Care Act 2014 and Mental Capacity Act 2005.
  • Excellent communication, planning, and problem-solving abilities.
  • A recognised qualification in Social Work, Occupational Therapy, Nursing, or equivalent (e.g. Level 5 Leader in Adult Care).
  • Commitment to multi-agency working and continuous professional development.

In person interviews will be arranged for Friday 12th September 2026.

The location base for this role is Aylesbury however this is a community role, so travel is required around the county. Therefore, you must have a full driving licence with access to a vehicle and be willing to travel county-wide as necessary.
