About the role

As a Social Work Assistant within the Home Independence Team, you will play a key role in supporting adults who require care and support services. You will work directly with individuals to identify their needs, strengths and desired outcomes, helping them maintain or regain their independence wherever possible.

Your responsibilities will include:

• Undertaking strengths-based assessments and reviews based on Buckinghamshire councils Better lives framework.
• Developing and reviewing person-centred care and support plans.
• Supporting individuals to achieve identified goals and outcomes.
• Carrying out risk assessments and promoting safe practice.
• Working collaboratively with families, carers, health colleagues and partner agencies.
• Monitoring and reviewing care arrangements to ensure they remain effective and appropriate.
• Maintaining accurate records and documentation in line with organisational requirements.
• Supporting individuals to make informed choices and remain in control of decisions affecting their lives.

You will be expected to work in accordance with the principles of personalisation, ensuring that the people you support remain at the centre of all decisions.

Working Pattern

This role includes:

• Monday – Friday 9.00-17.30 with Alternate weekend working Shifts between 7:00am and 10:00pm.

Mobile Working

This is a mobile role that offers flexibility.  You will travel to meet clients, carers and professionals across the county, whilst accessing digital systems and resources remotely through your laptop and mobile technology.

Essential Skills and Experience

• Experience working within Health and Social Care or a related setting.
• Experience undertaking assessments and identifying support needs.
• Knowledge of strengths-based and person-centred approaches.
• Experience developing and reviewing care and support plans.
• Ability to undertake and manage risk assessments effectively.
• Knowledge of safeguarding principles and promoting wellbeing.
• Understanding of manual handling practices.
• Experience transcribing and managing MAR (Medication Administration Record) forms.
• Knowledge of the Care Act and its application in practice.
• Awareness of CQC standards and expectations for domiciliary care providers.
• Ability to manage competing priorities within a busy and fast-paced environment.
• Strong written and verbal communication skills.
• Good IT skills and confidence using digital systems.
• Ability to build positive working relationships with clients, carers and partner organisations.

Essential Requirements

• Full UK driving licence.
• Access to a vehicle for work purposes.
• Willingness to travel county-wide as required.
• Flexibility to work alternate weekends and a shift pattern between 7:00am and 10:00pm.
