Location: Aylesbury

Overview

Do you have a passion for improving outcomes for children and young people with Special Educational Needs and Disabilities (SEND)? Are you looking for a rewarding role where you can make a meaningful difference to families, schools and communities every day?

As an Education and Healthcare Coordinator, you will play a vital role in coordinating statutory SEND services, ensuring children and young people receive the support and provision they need to achieve positive outcomes. Working closely with families, education settings and a range of professionals, you will help deliver high-quality Education, Health and Care (EHC) services across your allocated area.

This role offers the opportunity to make a genuine difference to the lives of children, young people and their families while working within a collaborative and supportive team dedicated to achieving positive outcomes for SEND learners

About us

Buckinghamshire Council is a relatively new organisation that combines the strengths and expertise of the previous five councils in the county to create positive change for local people, communities, and businesses. We are firmly focused on the future and have made a clear commitment to serve residents better and to provide stronger representation for Buckinghamshire, both locally and nationally.

About the role

This is an exciting opportunity to join a dedicated SEND service and contribute to improving the lives of children and young people with additional needs. You will be responsible for managing a varied caseload, coordinating EHC statutory processes and ensuring Education, Health and Care Plans are prepared, reviewed and monitored in accordance with relevant legislation and national guidance.

Working in partnership with parents, carers, schools, colleges and multi-agency professionals, you will act as a key point of contact throughout the assessment and review process. The role offers a great blend of case management, stakeholder engagement and problem-solving, requiring you to balance statutory deadlines whilst maintaining a strong focus on achieving the best possible outcomes for children and young people with SEND.

You will have the opportunity to build strong relationships across education and healthcare settings, support collaborative decision-making, and contribute to the development of inclusive practices within schools and colleges. Every day brings new challenges and opportunities to make a real impact.

Key Responsibilities

  • Coordinate and manage Education, Health and Care (EHC) statutory processes and casework.
  • Prepare and review EHC Plans in line with the Children and Families Act 2014 and SEND Code of Practice.
  • Manage a caseload across a designated geographical area, ensuring timely and professional delivery of services.
  • Monitor educational placements and provision to ensure EHC Plan requirements are being met.
  • Coordinate and lead meetings involving families, schools and wider professionals.
  • Review EHC Needs Assessment requests and supporting evidence to inform decision-making.
  • Develop and maintain positive relationships with parents, carers, education providers and partner agencies.
  • Prepare reports, case summaries and information for panels and decision-making meetings.
  • Support the resolution of issues, concerns and disagreements through effective communication and collaboration. For a full list of responsibilities and duties, please refer to the Job Description.

About you

You will bring experience of working within SEND, education or a related professional environment and have a good understanding of the legislation, policies and guidance that underpin SEND services.
